OBJECTIVE
To determine the scope, source, and mode of transmission of a multifacility outbreak of extensively drug-resistant (XDR) Acinetobacter baumannii.
DESIGNOutbreak investigation.
SETTING AND PARTICIPANTSResidents and patients in skilled nursing facilities, long-term acute-care hospital, and acute-care hospitals.
METHODSA case was defined as the incident isolate from clinical or surveillance cultures of XDR Acinetobacter baumannii resistant to imipenem or meropenem and nonsusceptible to all but 1 or 2 antibiotic classes in a patient in an Oregon healthcare facility during January 2012–December 2014. We queried clinical laboratories, reviewed medical records, oversaw patient and environmental surveillance surveys at 2 facilities, and recommended interventions. Pulsed-field gel electrophoresis (PFGE) and molecular analysis were performed.
RESULTSWe identified 21 cases, highly related by PFGE or healthcare facility exposure. Overall, 17 patients (81%) were admitted to either long-term acute-care hospital A (n=8), or skilled nursing facility A (n=8), or both (n=1) prior to XDR A. baumannii isolation. Interfacility communication of patient or resident XDR status was not performed during transfer between facilities. The rare plasmid-encoded carbapenemase gene blaOXA-237 was present in 16 outbreak isolates. Contact precautions, chlorhexidine baths, enhanced environmental cleaning, and interfacility communication were implemented for cases to halt transmission.
CONCLUSIONSInterfacility transmission of XDR A. baumannii carrying the rare blaOXA-237 was facilitated by transfer of affected patients without communication to receiving facilities.

An experimental study of the fracture mechanics and rheology of ice from the Ronne Ice Shelf, Antarctica, is currently being undertaken. The apparent critical stress-intensity factor (or apparent fracture toughness, KQ) for crack propagation has been measured using a three-point bend method for inducing crack growth perpendicular to the axis of cylindrical ice-core specimens. Tensile crack nucleation under applied uniaxial compressive stress has also been evaluated. Both methods have allowed a profile of ice elastic and fracture properties with depth through the ice shelf to be constructed. Preliminary results indicate that the measured elastic modulus increases with depth through the firn and upper meteoric ice before reaching a constant value in the deeper, dense meteoric and basal marine ice. The resistance to fracture, as measured by changes in apparent fracture toughness and crack-nucleation stress, increases with depth right through the firn and meteoric ice layers. A simple fracture mechanics model applied to the Ronne Ice Shelf indicates that crevasses form from small surface cracks, less than 40 cm deep, which quickly grow to depths of 40–60m and then remain stable.

Charcoal is the result of natural and anthropogenic burning events, when biomass is exposed to elevated temperatures under conditions of restricted oxygen. This process produces a range of materials, collectively known as pyrogenic carbon, the most inert fraction of which is known as black carbon (BC). BC degrades extremely slowly and is resistant to diagenetic alteration involving the addition of exogenous carbon, making it a useful target substance for radiocarbon dating particularly of more ancient samples, where contamination issues are critical. We present results of tests using a new method for the quantification and isolation of BC, known as hydropyrolysis (hypy). Results show controlled reductive removal of non-BC organic components in charcoal samples, including lignocellulosic and humic material. The process is reproducible and rapid, making hypy a promising new approach not only for isolation of purified BC for 14C measurement but also in quantification of different labile and resistant sample C fractions.

We discuss our rational approach to incorporate optically nonlinear molecules into polymeric and cross-linked materials through the use of isocyanate-hydroxy coupling chemistry. Thin film fabrication, optical loss, poling, second harmonic generation, and electro-optic properties are discussed.
